This is My agreement between Me and you and your children after you, which you must obey: Every man among you must have this religious act done.
But the man who has not had this religious act done, of cutting off his piece of skin, will be cut off from his people. He has broken My agreement."
Abraham and his son Ishmael had this religious act done that very day.
They prayed that good would come to Rebekah, and said to her, "You are our sister. May you become the mother of millions. May your children and all their children's children after them take over the cities of those who hate them."
Then God said to him, "I am the All-powerful God. Have many children and add to your number. A nation and a group of nations will come from you. Kings will come from you.
Kings will take care of you as fathers. And their queens will be your nursing mothers. They will bow down to you with their faces to the ground, and kiss the dust of your feet. Then you will know that I am the Lord. Those who wait for Me with hope will not be put to shame.
This was the promise God made: "About this time next year I will come, and Sarah will have a son."

But if he wash [them] not,.... Neither wash his clothes: nor bathe his flesh; if he is negligent, and does not take care to make use of these ablutions:
then he shall bear his iniquity; his guilt shall remain on him, and he shall suffer the punishment the law exposes him to, either by the hand of God, or the civil magistrate, which is due to persons that enter into the sanctuary in their uncleanness, or eat of holy things. For not washing his body the punishment was cutting off, and for not washing his garments, beating, as Jarchi says.
